Fosse's outreach services offer individuals the opportunity to maintain autonomy within the comfort of their own homes, empowering them to pursue independent living while receiving tailored assistance. This personalised support fosters an environment where individuals can thrive, providing the necessary tools and guidance to help them achieve their personal milestones and lead fulfilling lives on their terms.
Outreach is a service designed to help people with a wide range of support needs retain their independence by being supported in their own home. People in supported living have their own tenancy and are responsible for their own bills and cost of living. Together providers and clients come up with suitable time frames and hours of support throughout each week to ensure their needs are met.
Through our outreach services, we support adults of all ages with a wide range of needs, including learning disabilities, complex needs (including dual diagnosis), mental health conditions and substance abuse & addicton (Information around each of these can be found on the clickable links below).

From just an hour of support, to 24/7 bespoke provision, our services cater to a variety of needs and span across a range of support living settings including Individual homes, Flat schemes, Individual outreach services, Shared supported houses and Extra care services.
Our team has a wealth of knowledge and experiene including a band 9 Clinical Director, PBS (Positive Behavioural Support) Lead, Mental Health Recovery Star and Quality Assurance Manager to ensure we meet the highest possible standards across all of our support disciplines.
